Retain the default form name csdocsp. Make the changes to the form for Company 1 and save it in the Company 1 folder, then do the same for Company 2 and Company 3. If you're accessing Partner via a server, then save these forms in the company folders on the server.
Any images that have been added to each of the forms will need to be saved in their respective folders as well. As with Neville's comment above, once you have finalised the design for each form you will need to create a pdf for e-mailing purposes and again, save each one in the company's respective folder.
